timed delivery quests.  the NPC can deliver her OWN pies across the continent rather than ask me to walk, SLOWLY, without any ability to defend myself or attack or fly or run or anything, through the most dangerous areas in the game, just to make sure a blueberry pie doesn't fall.  Then comes the complaint from the destination NPC that the pie had spoiled while I was walking...

People whining to other people to mute their mics when you can simple mute them yourself. (Not saying you guys are bad, this just made me think of this)
People who play music through their mics. Half the time it's not even anything good, it's commonly rap music from my experience.

somthing along the same train of thought the fact theres very little way to test the quality of your mic during voice chat other than the reactions you get from others

to often i find that im talking to myself for an hour before i learn that I cant be heard
