When a boss takes down one of your party members, then you Phoenix down that member the boss attacks that same exact party member you just revived who only has 10% life from the revival and they get knocked right back down. This happens too often in Final fantasy games and wastes away my revival items, it's even worse if that party member is a healer.  :l

Billow Pillow

Another slightly annoying thing is that moment when you get trapped inside a spawn killing.   ovO

When you need to save an NPC in order to continue with the story, but if the NPC dies, nothing tells you that you needed the NPC or that you won't be able to progress.

"(Ticktock, time is running out) What are you doing now?
I don't know where you are, don't even know your name.
They think I'm crazy, my heartbeat goes up..."

It's even worse if the NPC's AI isn't very good. On dragon's dogma my sister's pawns kept jumping off of cliffs instead of taking the safe way down, and escort missions are horrible with some characters.  :c I wish they would make the AI smart enough to avoid common sense hazards like cliffs.

(I don't know how many times she had to save a pawn who knocked itself out from jumping off a cliff.)

Some games managed to perfect NPC AI like Alyx from half life. The first NPC AI you actually depended on , and thought was a member of your team. Also Claptrap was the first AI that gave you escort missions , and didn't drive you nuts.

If an NPC in Morrowind is important for the main quest and they die, the game tells you what just happened and suggests you reload.

When games have invincible NPCs who shouldn't be.

My dad and I hated letting the marines drive in one of the Halo games. Any time one would drive a warthog, they'd run one of us over. :l

In Halo 1 Marines could drive you to a waypoint or you could drive. I think I jumped on the back of the vehicle once and the Marine if I recall hit random rocks and tipped the vehicle over.  :I  Never again am I going to let the Marines drive me in that game.  ovO

Just like the taxi drivers in GTA IV.I press hurry , and bam.The Taxi flies up on to a helicopter which soon explodes causing the taxi to fall on the wing of a plane. The planes speed made the taxi fall , and it blew up on happiness island.I WASN'T EVEN AT THE ISLAND , AND MY WAYPOINT WAS IN THE OTHER DIRECTION.

I wish Oblivion and Skyrim did that. I wanted to try depopulating the entire world, but way too many characters just went unconscious  >:/

Gonna point out two:
MINECRAFT: When you are mining, everything is fine, you have your headphones plugged in all the way, then the sound errors, and you turn - Boom.
SKYRIM: When you're walking around, and a dragon just appears and kills you. ((Happened to me even right when the game started  >:/ ))

Ok. . . *Sigh* Alright.

This has happened to me on numerous occasions

Nala Valor

I don't know if it has been mentioned but... Sand-attack in pokemon stadium 1 and 2. After 2 sand attack, your pokemon cannot land one single hit. :I I can't hit anyone and then I get defeated.

Any accuracy lowering move in general really.
Don't know what to put here.
So I just am typing these words, not knowing what to type here.
Maybe later I'll figure out.

It's even worse in a game with a boss who has Mega Death. You have to hope it misses at least one of your party members, and hope you can revive the other two in time.  o_o

Yeah, you have to time those things carefully - i.e. use it only when you know another of your characters will be able to get a heal in before the boss attacks again.
